As part of the Data Protection Act you have a number of rights including:
- Being able to request access to view or to obtain copies of what information the practice holds about you. This is known as a Subject Access Request.
- You also have the right to have information about you amended should it be inaccurate.
Should you wish to make a Subject Access Request please complete and hand in this Subject Access Request Form to a member of staff and the admin team will soon be in contact with you. For some guidance on the application process check out our guide: Information Guide – Subject Access Request
Alternatively, you can submit an email request as long as you provide your name, date of birth, registered address and the related periods that you wish to obtain records for.
